Wound Care Nurse Practitioner (NP)

Job Description:

Old Pueblo Specialty Group is a locally owned specialty medical practice providing advanced wound care across multiple care settings. We are seeking a dedicated Wound Care Nurse Practitioner (NP) who is passionate about delivering holistic, patient-centered care. You’ll work independently and as part of a collaborative team to provide specialized wound treatment in patient homes, assisted living facilities, and rehabilitation centers.

Objective/Purpose:

The purpose of this role is to improve healing outcomes and reduce hospital readmissions by delivering high-quality, evidence-based wound care. The Nurse Practitioner will assess, treat, and manage complex wounds, support continuity of care, and serve as a key clinical partner to physicians, caregivers, and care facilities throughout the patient’s healing journey.

Qualifications:

  • Current and unrestricted Nurse Practitioner license in the state of Arizona

  • Master’s degree or higher from an accredited NP program

  • Certification in wound care (WOCN, WCC, or equivalent) preferred, or willingness to obtain

  • Minimum of 1–2 years of experience in wound care, home health, rehab, or geriatrics (preferred)

  • Valid driver’s license and reliable transportation

  • Strong communication and clinical documentation skills

  • Ability to work independently and manage a flexible field-based schedule

  • Compassionate, professional, and patient-focused

Job Responsibilities:

  • Perform in-home or on-site wound assessments and deliver advanced wound treatments

  • Develop and implement individualized care plans based on patient conditions and goals

  • Monitor wound progression and document findings in a timely manner using EMR

  • Collaborate with physicians, care coordinators, and facility staff for care planning

  • Educate patients, caregivers, and families on wound prevention and self-care

  • Assist in transitions of care, including discharge planning and coordination with home health providers

  • Ensure compliance with clinical guidelines, company policies, and regulatory standards

  • Provide referrals and coordinate with specialists or mobile PCPs as needed

  • Participate in quality improvement and outcome tracking initiatives
