Old Pueblo Specialty Group is a locally owned specialty medical practice providing advanced wound care across multiple care settings. We are seeking a dedicated Wound Care Nurse Practitioner (NP) who is passionate about delivering holistic, patient-centered care. You’ll work independently and as part of a collaborative team to provide specialized wound treatment in patient homes, assisted living facilities, and rehabilitation centers.
The purpose of this role is to improve healing outcomes and reduce hospital readmissions by delivering high-quality, evidence-based wound care. The Nurse Practitioner will assess, treat, and manage complex wounds, support continuity of care, and serve as a key clinical partner to physicians, caregivers, and care facilities throughout the patient’s healing journey.
Current and unrestricted Nurse Practitioner license in the state of Arizona
Master’s degree or higher from an accredited NP program
Certification in wound care (WOCN, WCC, or equivalent) preferred, or willingness to obtain
Minimum of 1–2 years of experience in wound care, home health, rehab, or geriatrics (preferred)
Valid driver’s license and reliable transportation
Strong communication and clinical documentation skills
Ability to work independently and manage a flexible field-based schedule
Compassionate, professional, and patient-focused
Perform in-home or on-site wound assessments and deliver advanced wound treatments
Develop and implement individualized care plans based on patient conditions and goals
Monitor wound progression and document findings in a timely manner using EMR
Collaborate with physicians, care coordinators, and facility staff for care planning
Educate patients, caregivers, and families on wound prevention and self-care
Assist in transitions of care, including discharge planning and coordination with home health providers
Ensure compliance with clinical guidelines, company policies, and regulatory standards
Provide referrals and coordinate with specialists or mobile PCPs as needed
Participate in quality improvement and outcome tracking initiatives
